Type: Tribal Salary Range: $56,378 to $84,566 / Per Year Open Period: 10/26/2015 to 10/30/2016 Summary: The Kewa Pueblo Health Corporation is established for the purposes of carrying out the vision and mission of the Santo Domingo Health Center, which is to provide community health services to the people of the Santo Domingo Pueblo.

POSITION PURPOSE The PRC Case Manager/RN is responsible for providing relevant case management, social and supportive services to the community of Santo Domingo as it relates to PRC/Purchased/Referred Care, patient registration/benefits and the SDHC Administration. This position works with multiple Departments in the clinic and coordinates care with Primary Care Practitioners as needed. This position works with hospital and rehabilitation center discharge teams, home health care agencies to assist in identifying vulnerable Pueblo residents and coordinating their transitional care utilizing all clinic resources to assure best medical outcomes. Position supports in-service training on relevant clinical topics. WORK ENVIRONMENT Work environment: The work environment characteristics described here are representative of those an employee encounters while performing the primary functions of this job. Normal office conditions exist, and the noise level in the work environment can vary from low to moderate. This position may be exposed to certain health risks that are inherent when working within a health center facility. Physical demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job. While performing the duties of this job, the employee may be required to frequently stand, walk, sit, bend, twist, talk and hear. There may be prolonged periods of sitting, keyboarding, and reading. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with patients, vendors and staff. Mental demands: There are a number of deadlines associated with this position. The employee must also multi-task and interact with a wide variety of people on various and, at times, complicated issues. Duties: ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES PRC Applications/Notifications:
• Assess patient condition and establish appropriate nursing needs. Identifies goals and nursing actions required to meet diagnosis-specific goals. Develops a care plan that addresses physical, psychological, social, and rehabilitative discharge planning to meet those goals.
• Collaborates with patient, family members, and health care team members with development, implementation, and evaluation of patient plan of care. Integrates cultural and age-specific differences when developing, implementing, and evaluating patient’s plan of care.
• Evaluates progress toward meeting patient care goals. Collaborates with patient, family members, and members of health care team to revise plan of care as needed.
• Utilizes existing quality-improvement systems to create constructive change. Participates in development, identification, implementation, and evaluation of clinic and medical center goals.
• Maintains strict confidentiality with regards to patient information.
• Utilizes health care team resources to maximize quality patient care.
• Educates patients or providers on the five (5) PRC eligibility factors.
• Other duties assigned. Personal Contacts and Inquires:
• Communicates with patients to coordinate medical appointments based on referrals, and follow up treatment plans as continuity of care. Qualifications: MINIMUM MANDATORY QUALIFICATIONS Experience:
• A minimum of five (5) years in the medical field/hospital setting with clinical experience in skilled nursing care and Education:
• Associate Nursing Degree and New Mexico RN license; Bachelor degree preferred; two years case management experience preferred. Basic Life Support (BLS) certification; CPR/first aid certification or must obtain within 30 calendar days of hire. Mandatory Knowledge, Skills, Abilities and Other Qualifications:
• Excellent computer skills and knowledge of software and applications associated with general office administration, excellent communication skills, written and verbal; able to work flexibly.
• Able to gather information from face-to-face interview or by telephone with patients, private contracted or open market providers such as Medical Records departments, Business Office personnel.
• Thorough knowledge of the Privacy Act of 1974, which mandates that the incumbent shall maintain complete confidentiality of all medical records and Contract Health patient individual files and other pertinent information, that comes to his/her attention.
• Knowledge of the PRC/Purchased/Referred Care program or ability to quickly learn including an awareness and understanding of the purpose, objectives, commitments and priorities to perform both recurring and special assignments.
• Thorough working knowledge of various Third Parties, such as Medicare, Medicaid, Private Insurance, etc., availability of health care resources, rules of eligibility for medical and other available programs to assist in payment of health care costs.
• Thorough knowledge of medical terminology and medical care practice and procedures.
• A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers.
• This position may be exposed to certain health risks that are inherent when working within a health center facility. PREFERRED QUALIFICATIONS
• Bachelor of Science in Nursing preferred; (BSN) two years case management experience preferred in a related field
• Bilingual skills in English and the Keres native language
