The Diabetes Coordinator provides diabetes services, training and education to clients and staff and develops all HHS policies, objectives, procedures and activities related to diabetes. This position also coordinates the development, implementation, administration activities and evaluation of the Mille Lacs Band of Ojibwe Diabetes Program. This position also provides clinical and community diabetes health education. Duties: ? Management of the Diabetes grant and program services. ? Writing and submission of grant applications, grant reports and other required documentation. ? Supervises the diabetes team and assists with scheduling patients/diabetes program activities. ? Provide diabetes teaching for Health Human Services staff and patients. ? Assist with the coordinating of the diabetes activities through collaboration with multiple departments. ? Is responsible for overseeing the planning, implementation and evaluation of the diabetes self- management program at all sites. ? Maintain and keep current with diabetes treatment changes and improvements. ? Set clinic dates, times and posts calendar. ? Schedules appointments for patients. ? Completes and submits appropriate paperwork for the providers and for billing purposes. ? Collects, organizes and reviews patient data regularly for quality assurance ? Documents and performs periodic on-going assessment for the individual patient’s diabetes education needs and progress and makes recommendations to the providers. ? Must be able to represent the Public Health department to Band members and outside parties in a professional and courteous manner. ? Must be able to communicate effectively with a variety of participants and other caregivers. ? Must maintain strict confidentiality and abide by HIPPA regulations. ? Provides diabetes classes and maintains ADA recognition for clinical diabetes program. ? Administer activities of grant application, reporting, budget development, program monitoring, program development and policy procedure development ? Oversee related duties of recruitment, retention, supervision, orientation, assignments and evaluation. ? Develop, review and select appropriate educational materials to be used with clients and for use in training health care professionals and paraprofessionals. ? Coordinate collaboration of program services with other community health resources e.g. Indian Health Service, other Tribal Health programs and community agencies. ? Provides comprehensive and general diabetes education and care to patients, their families and the community in multiple settings e.g. clinics, homes, community centers and schools. ? Provides both diabetes and nutritional education to health care professionals and paraprofessionals. ? Maintains appropriate program records in compliance with Indian Health Service, Tribal and granting agencies policies. Qualifications:
• Bachelor’s degree in nursing with a current, unrestricted RN license from MN State Board of Nursing and Certified Diabetes Educator credentials or willingness to obtain Certified Diabetes Educator credentials.
• OR
• Associate degree in nursing with a current, unrestricted RN license from MN State Board of Nursing and Certified Diabetes Educator credentials or willingness to obtain Certified Diabetes Educator credentials.
• Two [2] to three [3] years of experience in public health, chronic disease management or health education is preferred.
• Cardio Pulmonary Resuscitation [CPR] certified required.
• Experience with insulin pumps and management preferred.
• Able to work independently with minimum supervision. Capable of independent nursing judgment and decision-making.
• Experience with or has an understanding in diabetes and other chronic illnesses.
• Has 2-3 years of experience in program management.
• Excellent oral and written communication and organizational skills.
• Ability to keep detailed records.
• Possesses in-depth knowledge and understanding of health promotion and disease prevention and the ability to teach those principles to others.
• Capable of maintaining strict confidentiality of patient information.
• Ability to pay attention to detail and work under time constraints.
• Ability to teach health issues and lead discussions for large and small groups.
• Knowledge of local medical services available to appropriately assist clients.
• Computer skills in a windows environment including spreadsheets, Excel and Word processing.
• Experience in American Indian culture and able to demonstrate cultural sensitivity.
• Able to keep current with changes in policy, regulations and standard of practice in the field of nursing and with the diabetes population.
• Must have current driver’s license, reliable personal transportation, proof of current insurance coverage, and insurability under the Band’s Auto Insurance Policy.
