POSITION SUMMARY: The Family Nurse Practitioner (FNP) is responsible for providing medical care for ambulatory patients in a Family Practice/Primary Health care clinic. The FNP will set overall treatment and care protocols, guidelines, prevention goals and objectives, and has the responsibility for ensuring quality of care through a Quality Assurance process. The FNP along with the Medical Director provides information and assistance to the Health Services Administrator for reports to governing, licensing, accrediting, and funding agencies. This position requires some call beyond the usual clinic hours and attendance at mandatory meetings. FNP reports directly to the Medical Director but is expected to function autonomously in a variety of clinical settings. Duties: RESPONSIBILITIES AND DUTIES: 1. Provides diagnostic, preventative and therapeutic health services to patients and family members in a primary care facility, with the ability to function as an independent practitioner. 2. Assesses and examines patients, reviews past medical history, and requests and/or performs/recommends diagnostic tests and examinations deemed necessary to obtain all possible information related to each case and interprets the results. 3. Discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional, and mental problems. 4. Provides direct medical care in the presence of illness or disability in order to maintain life, provide comfort, reduce distress and enhance coping ability. 5. Makes decisions concerning medical care needs of patients which may involve various treatment modalities, medication prescription, and consultant/referral services when appropriate. 6. Provides surveillance over medical practices in order to stabilize chronically ill persons; adjusts treatments within established guidelines and recognizes when to refer the patient to the physician or other health team members. 7. Participates with other professionals and agencies involved in providing health care and services required for immunizations, disease control, environmental health, and health education. 8. Identifies risk management issues and brings to the attention of the Medical Director. 9. Works collaboratively with physicians in management of selected complex medical problems. 10. Provides limited range of emergency services or crisis intervention including life-saving emergency procedures in order to stabilize a patient until Emergency Medical personnel arrive or the patient is transported to a hospital facility. 11. Provides medical care and preventive health services to individuals, which may include guidance in nutrition, common illnesses, accidents, children’s growth and development and child rearing. 12. Refers patients to appropriate identified specialists. 13. Evaluates total health care needs of patients and develops or initiates plans to meet these needs; and teaches patients and other required information for use in program operation, quality assurance and to meet reporting requirements. 14. Must provide cross coverage at NATIVE HEALTH Central as needed. 15. Maintains knowledge and proficiency in family practice through continuing education, staff meetings, and workshops. 16. Participates in NATIVE HEALTH’S overall Quality Improvement program. 17. Will be required to rotate into call schedule for NATIVE HEALTH. 18. Perform other work related duties as assigned. Knowledge of the diagnostic, preventive and therapeutic modalities related to a variety of medical conditions that may include OB/GYN, Pediatric, Internal Medicine, mental health, and nutrition. 2. Knowledge of pharmaceuticals to safely prescribe, monitor and assess the effectiveness of the drug. 3. Knowledge of human behavior during stressful situations in order to exercise emotional control, tact, patience and reliability in carrying out patient care. 4. Knowledge of the availability and use of resources designated as primary care and referral services. 5. Knowledge of program planning concepts and practices. 6. Knowledge in health education theories, concepts, principles and practices specifically in diabetes, cardio vascular, cancer, stroke and pulmonary disease. 7. Knowledge of chronic care model. 8. Knowledge of the cultural, social, and economic backgrounds of the various ethnic or races in order to provide culturally appropriate care in the clinic setting. 9. Ability to collect, organize, record, and communicate, in a meaningful way, data relevant to primary health assessments including detailed family and medical history, physical examination, and laboratory or additional tests. 10. Ability to perform special screening and developmental tests, and routine or specialized laboratory tests. 11. Ability to provide written or oral presentations, and SOAP documentation. 12. Skills in written and verbal communication. 13. Ability to work independently and take initiative in addressing concerns and issues. 14. Ability to establish program goals and objectives and to assess progress toward their achievement. Qualifications: QUALIFICATIONS/EXPERIENCE/REQUIREMENTS: 1. This FNP position requires completion of an accredited Nursing Program, an unrestricted license as a Registered Nurse, and license as a Nurse Practitioner. 2. Current medication dispensing license (DEA). 3. This position requires advanced experience as a Licensed Clinician. Candidate must demonstrate a minimum of two years of clinical experience as a Licensed Clinician. 4. Medical experience in an outpatient family medical clinic including pediatrics, obstetrical/gynecological, medical care, and non-emergency care. 5. Bilingual in English/Spanish recommended. 6. Possess valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver license during employment. 7. Must pass a criminal background check with a Class I Fingerprint Clearance Card within FNP 3/3 the initial ninety (90) calendar days of employment. 8.
