Job Title: Community Health Worker Liaison
Terms: Full-time, Non Exempt
Supervisor: Direct Services-Shelter Director
Job Overview:
As a Community Health Worker, you will play a crucial role in improving the health and well-being of our community members. You will work closely with individuals and families to provide education, support, and resources to promote healthy lifestyles.
The Community Health Worker (CHW) will serve as a liaison between health care providers and community members ensuring that individuals have access to the resources they need to address their healthcare needs. You will conduct outreach efforts to engage with community members, assess their health needs, and provide them with information and referrals to human service programs and healthcare services. You will help enroll them into appropriate human service programs to eliminate barriers to their health by meeting Social Determinate of Health (SDoH) needs and assist them to remain living independently in the community in a health, recovery focused lifestyle. Additionally, you will collaborate with healthcare professionals and community organizations to develop and implement community health initiatives.
To excel in this role, you must have excellent communication and interpersonal skills. You should be able to build rapport and trust with diverse individuals and be able to effectively communicate information in a clear and understandable manner. Additionally, you should have strong organizational skills and the ability to manage multiple tasks and responsibilities.
1. Serve as a liaison between communities and health care agencies.
- Conduct assessments to identify community members' health needs and develop individualized plans to address those needs.
- Conduct outreach efforts to engage with community members and promote community health initiatives.
- Assist community members in navigating the healthcare system and accessing necessary resources.
- Maintain accurate and up-to-date records of interactions and interventions.
- Collaborate with healthcare and community agencies to coordinate and facilitate access to healthcare and services for community members.
7. Provide guidance and social assistance to community residents
8. Complete SDoH screening to identify and prioritize the individual’s SDoH needs.
9. Provide case management and follow-up to ensure that the linkages to services and supports addressed the participant’s needs.
10. Provide outreach and education to local service providers, stakeholders and individuals with SDoH needs within Indiana County through presentations at social service meetings, telephone connections with potential referral agents and the appropriate use other communications mediums for educating the community on these services.
11. Attend necessary trainings and meetings as required; travel required.
12. Promote practices that support equity, diversity, inclusion, and cultural humility.
13. Other duties and projects as assigned.
Qualifications:
· Project coordination and organization skills; ability to manage multiple projects with attention to detail; meet deadlines, ability to handle interruptions, and produce timely, accurate work.
· Previous experience in a community health or related role.
· Excellent written and strong communication and interpersonal skills.
· Good listener, thoughtful respondent and positive influencer.
· Ability to work independently and as part of a team.
· Excellent organizational and time management skills.
Education and/or Experience: Bachelor’s degree in related field preferred, or commensurate experience, previous experience in the human services, and social work preferred.
Computer Skills: To perform this job successfully the following computer skills are required: Computer Literate, including the Windows Operating System and Microsoft Word, Outlook, Excel, Internet Explorer; and data entry.
Conditions of Employment: The conditions of employment for this job description are:
· Current resident of Indiana County preferred or individual with strong personal and/or geographic connections to the communities served.
· Able to work evenings and weekends as needed to support the achievement of all project goals and objectives.
· Must possess a vehicle and valid driver’s license. Work travel, if required, is reimbursed at the federal reimbursement rate.
· Must have or be willing to obtain Act 33 & 34 clearances and FBI clearance with fingerprinting.
· Must be willing to obtain CHW certification; course to be provided by Employer. Must pass course or successful completion of course.

Pay: $14.69 per hour
