Position: Care Manager
Type: Full-time, hybrid
Travel: Required for community-based meetings
Work Setting: Remote (home office) with travel
What You’ll Do:
- Provide information and education to help Individuals understand the Medicaid waiver process
- Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
- Help Individuals access services and supports they need to meet their goals
- Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
Who You’ll Work With:
Care managers work with Individuals on the Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers
What You Need:
- A valid driver's license, car insurance, and reliable transportation
- RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
- The ability to pass thorough background screens
- Designated home office setting with computer, internet service, and smart phone
