Description:

This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, providing medication management per protocol, medication therapy monitoring, regulatory compliance, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.
