SUMMARY

The Community Health Worker (CHW) Program Manager will directly manage all CHW programming and initiatives to help impoverished communities of color with navigating complex healthcare systems, accessing and securing health insurance, manage and prevent chronic disease, and offer peer-delivered services to those in recovery. Will also manage the IUL Maternal & Child Health Program focusing on health issues concerning women, children and families.

The CHW Program Manager will manage part-time CHW’s to assist IUL program participants and Central Indiana community on addressing health disparities in communities of color. Assists with addressing the National Urban League’s (NUL) signature Project Wellness initiative, while enhancing the IUL’s CHW programs and services in alignments with the priorities and strategies of the Indianapolis African-American Quality of Life Initiative (IAAQLI).

Manages all local, state, federal, corporate and private departmental grants, and assists with writing grants and securing additional funding support. Prepares and submits monthly, quarterly reports and year-end reports to IUL Health & Wellness Director and Executive Vice President of Programs. Oversees all CHW, Doula and Lactation Specialist trainings for external referrals in partnership with IUL Training Works and various funders. Assists with identifying and securing external partnerships within the Indianapolis Health & Wellness Ecosystem. Manages data entry of participant demographics, enrollments, and provided services into Efforts to Outcome (ETO) database.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

§ Manage CHW and Maternal & Child Programs, services and staff;

§ Adhere to and closely manages program budgets;

§ Oversee CHW, Doula and Lactation Specialist online applications from general public and makes recommendations for training enrollments;

§ Track all CHW and Maternal & Child Program enrollments, activities, services, and client demographics;

§ Ensure CHW staff enters client demographics, program enrollments, and provided services are into Efforts to Outcomes (ETO) and other funder client tracking systems;

§ Work closely with Health & Wellness Director and EVP of Programs on identifying and developing programming related to prenatal and well-child care, nutrition, and infant maternal mortality prevention;

§ Assist with developing programming addressing child immunizations, child nutrition and services for children with special health care needs;

§ Review and approve program monthly narrative reports as submitted by part-time program staff and prepares project reports for senior management;

§ Assist with managing local, national and NUL sponsored CHW and Maternal & Child related activities and events;

§ Identify opportunities for professional growth and development of staff and self;

§ Represent the IUL on various local Maternal & Child related committees, taskforces, etc.;

§ Assist with conducting maternal and child forums, town hall meetings, and panel discussions;

§ Work closely with IUL Training Works Program staff on referrals, training enrollments, and securing credential attainments;

§ Work collaboratively with the IUL management team on fundraising efforts;

§ Provide marketing material and needs to IUL Marketing Director, and assists with updating the IUL website with CHW and Maternal & Child information, initiatives, events, etc.

§ Attend mandatory workshops, seminars or conferences as required by the IUL to further enhance skills, knowledge and capacity;

§ Must be able to maintain personal boundaries while providing services;

§ Must have a valid driver’s license and reliable transportation, and

§ Carry out other duties as assigned

SUPERVISORY RESPONSIBILITIES

Will manage CHW part-time staff.

EDUCATION and/or EXPERIENCE

Bachelor's Degree (B.A.) in Social Work, Psychology, or Human Services preferred from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.

LANGUAGE SKILLS

Knowledge of Spanish desirable but not required.

READING/COMPREHENSION SKILLS

Ability to read and comprehend instructions, correspondence, and memos. Ability to write correspondence. Ability to effectively present information in one-on-one, small group and large group situations to customers and clients.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
