What are the responsibilities and job description for the Transitional Housing Case Coordinator position at Individual and Family Support Network (IFSN)?
AGENCY BACKGROUND:
Individual and Family Support Network (IFSN) has been moving people out of homelessness in Sonoma County since our inception in 1989. We do this by empowering families and individuals who are experiencing homelessness by providing them with rapid rehousing and transitional housing and a range of services that includes tenant counseling, financial assistance and education, free psychotherapy, and case management. Services are offered utilizing the Housing First and Harm Reduction models.
SUMMARY OF THE POSITION:
Reporting to the Program Manager, the Transitional Housing Program (THP) Support Specialist is a direct service position that works with clients directly referred by the Sonoma County Probation Department. The THP Support Specialist is responsible for assisting clients, residing in IFSN’s transitional houses, with life skills training, employment resources, budgeting, permanent housing and other resource linkage as needed. The goal of this program is to increase client self-sufficiency, improve life skills and decrease recidivism back into the criminal justice system and/or homelessness.
ESSENTIAL DUTIES
- Upon intake, the THP Support Specialist co-creates an individual Housing Plan to identify client barriers and create action steps to combat said barriers.
- Leads bi-weekly individual case management sessions along with weekly house meetings.
- Completes HMIS data entry and client case notes within 48 hours of client interactions.
- Maintains documentation and program participant files in accordance with agency and funding requirements.
- Works closely with clients to apply for public benefits.
- Reports any maintenance issues directly to THP Program Manager and/or Facilities Manager.
- Manages conflict or difficult situations constructively and seeks appropriate assistance.
- Regularly seeks to update and maintain current level of knowledge relative to professional practice.
- Develops an understanding of resources available in Sonoma County for the re-entry population, homeless individuals and registered offenders.
- Assist clients with employment services including, but not limited to, referrals to Job Link, assistance with resume building and mock interviews.
- Connects clients with local food resources.

QUALIFICATIONS
- Experience working with individuals who have lived experience with homelessness, and other marginalized populations in crisis.
- High level of organization and strong time management skills.
- Strong written and verbal communication skills.
- Ability to work effectively with people of diverse cultures, ages, economic backgrounds and abilities in a culturally competent and sensitive manner.
- Knowledge and experience with trauma-informed care, motivational interviewing and substance abuse recovery preferred.
- Knowledge of Housing First principles and methods, Harm Reduction, and Trauma Informed Care service delivery.
- Ability to work under pressure with a positive, solution-oriented approach.
- Computer literacy required, including experience with Microsoft 365 and Excel.
- Strong written and verbal communication; organization, conflict resolution and problem-solving skills.
- Excellent interpersonal skills, and cooperative, friendly, proactive, and helpful attitude with clients and co-workers.
- Must pass DOJ background clearance (fingerprinting).
- Automobile, valid driver's license and ability to meet IFSN’s driving policy.
- Bilingual (English/Spanish) preferred, but not required.
- Must pass security clearance
EDUCATION
B.A./B.S. in related field, and at least two years of related experience, or equivalent combination of education and experience.

BENEFITS:
Rate of pay: $24 -$26 per hour, DOE

Job Type: Full-time with the following alternative work schedules available:
9/80, 4/10s, 5/8s, 4/9s with 1 half day
Clinical Supervision provided for individuals pursuing Licensures: LMFT or LCSW
Possibility of embedding clinical accruing hours into the work week
