Job Title: Crisis Services QMHP-A/T
Supervisor:
Clinical Director/Licensed Mental Health Professional
Description:
Provide mobile crisis response/community stabilization/23-hour community stabilization services to individuals 18 years and older. Responsible for development and implementation of individualized services plan in accordance with program description.
Responsibilities:
- Provide early intervention mental health crisis services that are available 24 hours a day, 7 days a week.
- Provide direct services to clients, including linkage, advocating, supporting, and providing education as needed.
- Prepare written individual service plan, incorporating input from clients, involved service providers, and clearly identifying problem areas and needs, strategies, and service objectives.
- Link clients with external programs or services, such as health services, recreational activities, childcare services, financial resources, employment resources, childcare, AA/NA groups, transportation resources, and others
- Collaborate and coordinate with other service providers and professionals, such as attorneys, social workers, and probation/parole officers.
- Complete all progress notes according to ISBS policies and procedures.
- Recommend appropriate services for client.
- Assure client is informed of his/her human rights.
- Conduct discharge planning in accordance with program description.
- Attend all meetings as required.
- Provide accurate time sheets.
- Advise Clinical Director of the state of clinical services, its staff, and its needs and solicits his assistance in implementing solutions to problems.

Minimum Qualifications:
Must hold at minimum a Bachelor’s degree in Health and Human Services to include social work, gerontology, psychology, psychiatric rehabilitation, special education, sociology, counseling, vocational rehabilitation, criminal justice and human services counseling, or a related field from an accredited college or university and experience providing mental health services and case management to individuals suffering from a mental illness is preferred.
Must be credentialed by the Virginia Department of Health Professionals as a Qualified Mental Health Professional - Adult (QMHP-A) or Trainee (QMHP-T/A)
This position also requires a valid driver’s license, automobile and telecommunications availability, excellent verbal and written skills; counseling and social work theories and techniques; and community resources. Strong writing and case conceptualization skills and reference to a methodological. The successful completion of a criminal history or background investigation is required.
Job Types: Full-time, Part-time
Pay: $24.00 - $29.00 per hour
Expected hours: 20 – 40 per week
