Infinity Case Management's purpose is to provide quality care management services to meet all social and medical needs for senior adults and people with functional and/or cognitive disabilities through the Community Care Service Program (CCSP).

The Case Manager plays a crucial role in supporting individuals and families by assessing their needs, developing personalized care plans, and coordinating services to promote overall well-being through the Elderly Disabled Waiver Program (CCSP) funded by Medicaid. This position requires a compassionate and proactive approach to help clients meet their health and social needs. The ideal candidate will possess strong interpersonal skills and the ability to work collaboratively with various Medicaid Providers and other professionals within the community to achieve positive outcomes for clients.
Responsibilities
- Develop and implement individualized care plans based on assessment findings.
- Provide ongoing case management support, including monthly telehealth contacts and quarterly home visits.
- Facilitate access to resources within the community.
- Collaborate with community organizations, service providers, and government agencies to coordinate care.
- Provide problem solving tips to address conflicts effectively through conflict-resolution management strategies and interventions.
- Maintain accurate and up-to-date documentation of client interactions and progress in accordance with agency policies.
- Advocate for clients' rights and needs within the waiver services.
Minimum Job Qualifications
Education: Bachelor's degree in Social Work, Sociology, Psychology, Healthcare field or a related field.
Previous Experience: Two years experience in the human service or health related field.
Knowledge/Training:
- Ability to effectively coordinate and communicate with clients, service providers, general public, and other staff members.
- Possess skills in establishing and sustaining interpersonal relationships.
- Knowledge of Gerontology.
- Skills in team building and group dynamics.
- Knowledge of community organization and service system development.
- Problem solving skills and techniques.
- Knowledge and skill in social and health service intervention techniques and methodology.
Computer Skills: Must be computer proficient and possess experience with various computer operating systems. Must be able to quickly learn specific software and new applications.
Transportation: Must have reliable transportation. Travel is required in designated areas to visit clients.
