Position Summary : Provide oversight and supervision of behavioral health facilitators and patient engagement specialists, assisting in coordinating communication between patients and ARNP / Psychiatrist ensuring consistent patient care and coordination of services.
Essential Duties and Responsibilities :
- Practices exceptional customer service to all
- Supports the mission, vision, and values of Infinity Health
- Responsible for hiring and oversight of facilitators and patient engagement specialists
- Assess and organize needed training for facilitators and patient engagement specialists
- Coordinate facilitator absences, replacements and ensure back up plans are communicated to front desk and / or other concerned parties
- Participate in quarterly Behavioral Health CQI meetings to come up with goals and identify needs
- Complete facilitator QI goals, tracking and implementation
- Complete performance evaluations for supervisees
- Facilitate Tele-med appointments to include rooming patients, obtaining vitals reconciling medications, updating / reviewing charts, charting QI, completing re-call plan for next appointment
- Assist with schedule patients for ARNP / Psychiatrist when needed
- Administer injections as licensed
- Field patients calls and questions, relaying messages to ARNP / Psychiatrist
- Monitor court reports of patients
- Attend monthly managers meetings
- Assist with the implementation and sustaining of patient centered medical home status as needed
- Other Duties as Assigned
Qualifications : LPN or RN license, minimum of 6 months experience as Behavioral Health Facilitator preferred.
