Qualifications/Skills:
- Graduate of a Medical Assistant certificate program required
- Palliative Care/Hospice/Home Health experience preferred
- Expert customer service ability
- Assist with education of the community regarding Nevada Care Connect, NCC House Calls and associated services
- Possesses an ability to maintain good interpersonal relationships with patients, families, co-workers, and other health team members
- Learns and understands the roles of all members of the care team
Definition:
The Care Coordinator is responsible for driving the intake process and communicating patient needs. The Care Coordinator strives forcommunication excellence by bridging the gap between patients, caregivers, and all members of the care team.
This person builds a trusting relationship with patients and caregivers by understanding their critical needs and conveys that information to the care team. The Care Coordinator plays a vital role and is integral at every hand-off throughout the patient journey.
Responsibilities:
Perform all patient care coordination needs (including):
- Process all incoming referrals
- Conduct initial intake and review health history with patients and family
- Enter and update demographics, ensure practice forms are signed, and document time working in chart
- Follow-up on all tasks such as; open orders for e-prescribe, medication refills, DME, laboratory, specialist referrals, home health, follow up with patients across care transitions, and assist in entering medications into EHR
- Requests H&P
- Educates patient and family on how to access patient portal and view care plans
- Follows-up to obtain any missing practice forms
- Assist to access, scan and maintain medical records
- Notify care team members when visits are not complete
- Verify visit notes are complete with-in 24th day of visit
