Responsibilities:
The Hospice Aide reports to the Director of Patient Care Services or designee and works under the direct supervision of the RN Case Manager.
Functions:
- Receive the CNA assignment which directs frequency of care and care duties to be performed
- Maintains visit frequencies as indicated in the Plan of Care.
- Assist with personal care of patient, i.e. bathing, feeding, bed making, mouth care, vital signs and transferring
- Document in Care Plan Charting software.
- Keep Clinical Coordinator/Nurse Case Manager informed of condition of patient/family.
- Observe infection control and safety measures
- Assist with patient/family teaching concerning patient care and care for the caregiver.
- Bereavement activities as requested and/or able.
- Clean and monitor supplies.
- Provide companionship
- Assist with laundry, shopping, light housekeeping, meal preparation and other homemaker tasks
- Participate in staff and patient review meetings as requested.
- Work as an effective Infinity Hospice Care Group member.
Qualifications
Certified Nurse’s Aide license within state of practice.
- Prefer a minimum two years of CNA experience, one year in either a Home Health and/or Hospice setting recommended.
- Interpersonal skills to interact with patients, their families and staff.
- Strong keyboarding and computer skills, preferred.
- Ability to communicate clearly in English verbally and in writing
Physical Requirements/Work Environment:
- Must be able to lift, move, push, and/or pull objects, including the human body.
- Must be able to walk and sit for long periods of time.
- Must be able to bend, stoop, stand, push and pull
