Responsibilities:
The Nurse Practitioner assists with the medical and nursing component of Infinity Hospice Care Patient Care Services. This position reports directly to the Medical Director and Hospice Manager. The Nurse Practitioner works in conjunction with the Medical Director, Hospice Manager and Clinical Coordinator and the members of the Interdisciplinary Group to:
1. Obtain medical/health histories and perform physical examinations
2. Diagnose, treat and monitor acute and chronic diseases in collaboration with hospice physician and in accordance with the plan of care
3. Order, perform and interpret diagnostic studies in collaboration with the hospice physician
4. Identify, implement and evaluate appropriate individual plans of care, including pharmacologic interventions within the guidelines of the ARNP Practice Act of the state in which the hospice is licensed
5. Promote positive health behaviors and self-care skills through education and counseling.
6. Act as a care manger of the hospice patient by assessing and modifying the plan of the care with the Interdisciplinary team
7. Performs Initial Nursing Assessments as assigned
8. Assists the RN Case Managers with PRN nursing visits as assigned
9. Provide comprehensive functional assessments that include activities of daily living, instrumental activities of daily living, economic, environmental, mental status and social assessments
10. Provide care in multiple sites that include but are not limited to the patient's home, assisted living facilities, group homes, hospital and nursing homes
11. Provide accurate and appropriate documentation of patient/family services as specified in policies and submits this documentation in a timely manner
12. Participate in the team responsibility of providing 24 hour a day/7 day a week patient care
13. Act as a liaison between hospice and other professionals including physicians, hospitals, nursing homes, resource agencies and the medical community
14. Demonstrate knowledge of Infinity Hospice Care policies and procedures and pertinent State and Federal Rules and Regulations
15. Recognize, reinforce and participate in the Interdisciplinary Group Meetings
16. Support the mission of Infinity Hospice Care through formal and informal education
17. Participate in continuing professional development and maintenance of clinical skills
18. Act as a health care resource, interdisciplinary consultant and patient advocate
19. Participate in CQI activities
20. Assist with employee health activities
21. Responsible for monthly and as needed follow up with Outreach Program patients
22. Within 30 days of patient’s recertification in third or later benefit period, performs Face
to Face visit to determine the patients eligibility for recertification and documents the findings
23. Provides after hours on-call services via telephone, as assigned
24. Perform other duties as assigned
25. This position is paid by visit.
Qualifications
1. Current license as an Advanced Registered Nurse Practitioner in the state in which the hospice is licensed.
2. Bachelor's Degree in Nursing.
3. Minimum of two (2) years nursing experience.
4. Minimum of two (2) years experience as an ARNP with experience in gerontology experience with pain and symptom management is preferred.
5. Must have reliable transportation which will enable the ARNP to perform tasks and
responsibilities in a timely and appropriate manner. Must provide proof of automobile insurance.
6. Strong verbal and written communication skills.
