Overview
We are looking for a dedicated and empathetic Hospice Medical Social Worker to become an integral part of our team in La Grande. This role is pivotal in providing emotional, social, and practical support to patients and their families navigating the complexities of terminal illness.
The successful candidate will:
· Offer counseling and emotional support to help patients and families cope with grief, loss, and the emotional challenges associated with end-of-life care.
· Collaborate with an interdisciplinary team, including physicians, nurses, and other healthcare professionals, to develop and implement personalized care plans.
· Facilitate communication among family members and healthcare providers to address concerns and mediate conflicts.
· Coordinate treatments, services, and resources to enhance the patient's quality of life.
· Advocate for patients' needs by connecting them to appropriate resources and support systems.
· Assist families in managing stress and hardships related to terminal illness, including financial difficulties.
Qualifications:
· Master's Degree in Social Work
· Proven experience in a social work capacity
· Reliable transportation is required

Pay: $32.00 - $42.00 per hour
