Key Responsibilities:
- Assist the Specialty Care Team in coordinating safe, effective, efficient, equitable, patient-centered care.
- Assess the self-management skills of patients and caregivers, promoting wellness and autonomy through support, training, and coordination of referred services.
- Communicate regularly with patients and caregivers regarding referred services, utilizing sound knowledge and judgement to prioritize patient referrals, outreach, and coordination of care.
- Support care coordination within and outside of our health system, ensuring a consistent, effective, supportive system of care.
- Tracks and maintains case management coordination data, working autonomously to assess frameworks and identify solutions at an individual, community, and system level.
Requirements
To succeed in this role, you will require:
- High School Diploma or equivalent.
- Associate's degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field.
- 4 years of relevant professional-level experience may be exchanged for a degree.
- Basic Life Support certification required.
- Two or more years of experience in a healthcare setting, preferably in primary care or specialty clinics.
- Knowledge of in-depth medical terminology, safety, and infection control principles.
- Strong technical skills, including proficiency in computers and medical equipment.
- Excellent problem-solving, conflict resolution, interpersonal, and organizational skills.
