RN Case Manager

Job Title: RN- Case Management

Location: Orange, CA 92868

Duration: 6 monthswith possible extension to long term

Shift hours: 8:00am to 5:00pm (M-F)

Start: ASAP

Job Duties And Responsibilities:85% - Care Management Responsibilities:

• Needs Assessment: Use standardized health assessments to evaluate members' physical, functional, psychological, and social status, along with cultural, linguistic needs, and caregiver resources.
• Post-Discharge Assessment: Conduct assessments after hospital or emergency department discharges to identify the member’s needs, including follow-up care, medical equipment, and community resources.
• Care Plan Development: Create and implement personalized care plans with SMART goals. Regularly review and update the plan, assess progress, and identify barriers.
• Care Coordination: Ensure collaboration with the member’s family, healthcare providers, specialists, and community agencies. Facilitate referrals to behavioral health services, LTSS, and other community supports.
• Interdisciplinary Team Involvement: Participate in team meetings, collaborate with internal staff for case resolution, and escalate cases for higher-level review when necessary.
• Advocacy: Advocate for the member’s needs in terms of funding, treatment options, and care coordination. Continuously evaluate progress and goals.

Minimum Qualifications:

• Associate degree in nursing (ADN) or related field required PLUS 3 years of clinical experience and/or managed care experience required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN) degree or related field.
• Case Management Certification (CCM).
• Bilingual

Required Licensure / Certifications:
Active RN License

Pay: $43.66 - $69.86 per hour

Expected hours: 40 per week

Schedule:

• Day shift
• Monday to Friday

Willingness to travel:

• 25% (Preferred)
