Job Summary:
We are seeking a passionate and driven Bilingual Health and Wellness Coach fluent in English and Spanish to join our team. This role is ideal for someone who thrives in a community-focused environment and is committed to improving the health outcomes of diverse populations. The ideal candidate will have a background in personal training or recreational wellness, nutrition, community health outreach, and experience with health plans, including Medicare and Medicaid. Familiarity with Electronic Medical Records (EMR), specifically eClinicalWorks (ECW), is highly desired.
Key Responsibilities:
- Provide culturally competent coaching and education to clients in both English and Spanish, focusing on nutrition, fitness, stress management, and lifestyle improvement.
- Conduct community outreach activities to engage underserved populations and promote wellness initiatives.
- Collaborate with healthcare providers, case managers, and social workers to create and implement personalized wellness plans.
- Support patients in navigating their health plan benefits, with a special focus on Medicare and Medicaid programs.
- Develop and distribute culturally relevant educational materials and resources.
- Participate in health fairs, screenings, and other outreach events as a representative of the organization.
- Monitor and track patient progress, making necessary adjustments to wellness plans.
Qualifications:
- Fluent in English and Spanish (spoken and written) — required.
- 2–3 years of experience in personal training, wellness coaching, or recreational wellness activities.
- Proven experience in community health outreach programs.
- Solid knowledge of nutrition, healthy eating habits, and behavior change strategies.
- Experience with Medicare/Medicaid health plans and patient education.
- Proficiency in using Electronic Medical Records (EMR), especially eClinicalWorks (ECW).
- Strong interpersonal, motivational, and communication skills.
- Certification in health coaching, personal training, nutrition, or a related field is preferred (e.g., ACE, NASM, CHES, or similar).
- CPR/First Aid certification preferred.
Salary : $23 - $26
