Brief Description
The primary purpose of the Neighborhood RN Supervisor position is to utilize nursing knowledge and assessment skills in the development and implementation of individualized nursing care plans to ensure that customer needs and all applicable regulations are met. The RN Supervisor will also assist in the orientation and supervision of clinical staff, attend to the daily operations of assigned neighborhoods and assume a leadership role. The RN Supervisor is responsible for ensuring the continual delivery of high quality nursing care by adopting and implementing innovative nursing practices to improve our facility.
- Responsible for completing assessment for admissions and readmissions
- Initial and ongoing assessment of residents with changing conditions
- Assess learning needs of residents and families and carries out teaching as indicated
- Maintain ongoing communication with primary care providers (PCP)
- Provide timely and ongoing information for MDS assessments
- Perform urinary catheter (Foley and suprapubic) changes
- Performs peripheral and central line management and infusions
- Acquire knowledge of and clinically support current Interdisciplinary Team treatment interventions
- Communicate care plan changes with all providers
- Familiar with the types of events and the time frames in which these events should be reported to the regulatory agencies
- Notifies Administrative RN Supervisor about reportable events immediately
- Provide mentoring to staff who directly reports to this position.
- Develop and implement Performance Improvement Plan (PIP) and follows up on progress, and revises PIP as needed
- Ensure optimal level of care is provided)
- Use the concepts of Person-Centered Care to ensure all services are performed according to documented resident preferences
- Develop and maintain care plans
- Enter Physician orders into the electronic medical record
- Complete and review resident assessments
- Complete and review nursing progress notes
- Complete and review incident and accident report, and investigation forms
- Complete and review 24-hour report
- Respond and document as appropriate all activity related to clinical emergencies (Stat 5)
- Complete and review Medication Recapitulation Orders
- Active licensed Pennsylvania RN
- Bachelor of Science in Nursing degree from an accredited institution
- Current CPR/BLS required
- Certification in Wound Care is preferred
- Certification in Infusion Therapy is preferred
- 3 years of long-term care management experience required
- 2 years of acute care experience required
