Qualification and Education Requirements
- Bachelors or Master’s Degree in Social Work from a State accredited program
- License in Social Work in the state you will be working
- Valid driver’s license and car insurance
- One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred
Essential Duties
- Assess patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
- Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities related to legal and fiscal issues
- Facilitate community resources based on patient/family needs
- Participate in all mandatory staff in-services and meetings
- Maintain compliance with internal annual education requirements
- Timely submission of all documentation
- Conduct all business activities in a professional and ethical manner
- Performance of other duties as required
- Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
