Job Requirements
Nursys verification within 30 days prior to start. For any disciplinary actions please complete FACIS Questionnaire available in EII document library and upload with board documents and verification.
Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals. For any disciplinary actions please complete FACIS Questionnaire available in EII document library and upload with board documents and verification.
Pharmacology 1 or 2 exam required to be completed prior to start and updated every 3 years thereafter. Minimum 80% score. Only 2 attempts allowed.
Completed prior to first assignment and updated every 3 years thereafter in specialty. Minimum 80% or the national benchmark passing score that demonstrates competency in the area of specialization. Only 2 attempts allowed.
Completed prior to start and updated every 3 years.
HIPAA, Joint Commission -NPSG, OSHA Topics. Completed within a year of start updated yearly. Minimum 80% score. Only 2 attempts allowed.
Statement of good health or completed physical exam prior to first assignment, updated annually thereafter
1st PPD within 1 year, 2nd PPD within 30 days of start. And / or Negative IGRA within 90 days of start date lab printout prior to start. TB Questionnaire must be updated annually. If positive TB record; Requires Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire.
Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider. If negative, provide proof of two doses of Varicella vaccine, four weeks apart. No declinations accepted.
Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted. If Negative titer, administer two doses of MMR vaccine, four weeks apart.

Must be AHA.
Valid 10 years from the administered date. No declinations or attestations accepted.
