What are the responsibilities and job description for the Home Health LVN Shift Care position at Inland Valley Home Health Agency?
Possible Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans
- Perform ventilator care and trach care
- Perform catheter care
- Provide bathing and dressing assistance
- Conduct assessments and provide nursing care for individuals
- Perform wound care as needed
- Knowledge on how to use a hoyer lift or trainable
- Maintain accurate and up-to-date medical records using electronic health record systems such as Kinnser/Wellsky
- Able to lift, position, or transfer patient
- Able to lift and/or move supplies and equipment
Requirements:
- Valid Licensed Vocational Nurse (LVN) license in the state of California
- Experience providing total care to a patient on a one on one basis
- Proficient in using electronic health record systems, such as Kinnser/Wellsky, to document patient care
- Strong communication skills, both verbal and written
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
- Familiarity with medical records management and confidentiality protocols
- Knowledge of primary care principles and practices
