TEAM LEADER-EAST CENTER

POSITION SUMMARY:  Assist coordinator and supervisor in various administrative duties and provide support and assistance to residents of Eastern Care.

 QUALIFICATIONS:  High School Diploma and 2 years’ experience. Must be 21 years of age or older and possess a valid KY driver license.

 SKILL SETS:  Communication skills, dependable, consumer service (manages difficult client situations and responds promptly), team player, time management, well organized, work under stress, process verbal and written instructions, ability to problem solve.

PHYSICAL AND ENVIORMENTAL DEMANDS:  Talking and hearing required to communicate with clients, family members and other staff. Visual acuity needed to provide behavioral observations of clients. Manual dexterity for written communication and using office equipment. Possible exposure to infectious and/or communicable diseases.

 JOB SPECIFIC FUNCTIONS: 

1. In the absence of the Manager and supervisor, assists with administrative tasks as directed by supervisor.
2. Completes monthly required safety drills (fire, tornado & earthquake)
3. Maintains client schedule and chore list
4. Conducts phone screenings and coordinates with therapist and/or manager possible admission
5. Maintains offices in an orderly manner to ensure efficient program operation.
6. Provides support to clients as needed
7. Facilitate treatment groups
8. Complete checks of rooms and chore assignments to ensure timely and accurate completion
9. Provides reports on day to day operations as needed
10. Provides transport for clients in a Communicare vehicle as needed
11. Provides linkages to community resources for residents
12. Assume responsibility for quality client care
13. Arranges transportation for individuals to and from medical appointments, shopping and other events.
14. Manages work schedule to ensure adequate productivity.
15. Participate in agency continuing quality improvement (CQI) activates.
16. Perform other duties as assigned.

 MANDATORY REQUIREMENTS:

1. Adheres to Communicare, Inc. policies and procedures.
2. Attends mandatory in-services and other training/educational programs as required.
