Opportunity Overview We are seeking a licensed social worker to join us as a partner in delivering comprehensive, patient-centered care. Key Responsibilities
Conduct psychosocial assessments to identify the needs and concerns of geriatric patients and their families.
Provide counseling and emotional support to patients and their caregivers.
Assist patients in accessing community resources, such as support groups, financial aid, and home care services.
Collaborate with InMotion Home Therapy’s clinical team to develop and implement individualized care plans.
Advocate for patients to ensure they receive the necessary services and support.
Educate patients and families about coping strategies, self-care techniques, and available community programs.
Qualifications
Licensed Social Worker (LCSW) with a valid license in NJ.
Experience working with the geriatric population, preferably in home-based or healthcare settings.
Strong interpersonal and communication skills, with a compassionate approach to patient care.
Ability to work collaboratively as part of a multidisciplinary team.
Familiarity with community resources and healthcare systems that serve the elderly population.
