What are the responsibilities and job description for the Youth Care Coordinator position at InnerSourced Solutions?
InnerSourced Solutions has an immediate opening for a full-time Youth Care Coordinator to work with youth (ages 6-18) and their caregivers to identify needs and goals that will strengthen the family and the youth's future. Areas of focus include education, mental health, physical health, economic needs, and positive social development. Youth and families are met with weekly on average and work towards these goals together in the home, school or community.
Priority will be given to candidates with verifiable prior experience providing case management services.
REQUIREMENTS:
- Bachelor's degree in a mental health field (i.e, psychology, social work, human services) and 1 year of mental health experience, including mental health peer support or a Bachelor's degree in a field other than mental health and 2 years of verifiable mental health experience, including mental health peer support. This is NOT optional!
- Familiarity with educational and child welfare services, local community resources and support programs.
- Strong verbal and written communication and collaborative skills.
- Proficient with technology, as we use an electronic medical records system, (Insync).
RESPONSIBILITIES:
- Provide in-person case management services to children and adolescents based on authorized level of care.
- Provide linkages to programs and services that include but are not limited to mental health services, housing, entitlements (including insurance), etc.
- Monitoring and documenting progress to ensure effective care.
- Work closely with youth, caregivers, and behavioral health staff.
- Obtain initial and concurrent authorization for services
- Conduct intake assessments to evaluate client risk and needs as well as completing all initial documentation required by COMAR regulations for referred clients within appropriate time frames.
- Complete initial and ongoing plans of care.
- Coordinate, facilitate, and engage in family team meetings.
- Providing crisis intervention and immediate support to clients/families during periods of acute distress or mental health emergencies. And, may help clients access emergency services, devel coping or reinforce coping strategies, or stabilize mental health.
- Identify and provide supportive services to clients, and follow up.
- Maintain timely documentation within EMR (Insync) for each client contact, including but not limited to: contact, encounter and collateral notes, referrals, forms and applications, monthly reports, etc.
- Advocating for clients and collaborating with other professionals to help them navigate complex systems and access resources they need to thrive.

Proficiency with technology is essential, as we are a fully electronic environment. Applicants should be proactive, recognizing that in a direct service setting, work can vary in pace and will therefore be required to engage in essential tasks that include but are not limited to marketing and community outreach, chart reviews and accreditation projects during quieter periods.
