POSITION SUMMARY:
The Patient Services Coordinator (PSC) is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. As the Patient Services Coordinator, you will also assist with day-to-day office and staff management as directed by the Clinical Operations Manager.
What you'll do:
- Manages schedules for all assigned patients; edits schedule timely when patient visits must be reassigned.
- Answers calls and answers questions regarding patient and caregiver schedules or resolves/routes problems; refers clinical questions to Clinical Director as necessary.
- Completes requested schedules for all add-ons and applicable orders:
- Schedules Admission, Recertification, Resumption, and Discharge visits
- Follows up with clinician(s) on missed visits to ensure the visit is truly missed or if it can be rescheduled
- Reschedules declined or missed (if appropriate) visits
- Processes reassigned and rescheduled visits
- Ensures Supervisory Visits are scheduled according to branch protocol and regulatory mandates
- Prepares weekly clinician schedules; performs initial review of weekly schedule for productivity/geographic issues and forwards to clinician(s) for confirmation; sends schedule to Assistant Clinical Director for approval prior to distribution to staff if productivity is not being met.
- Ensures adequate staffing coverage for vacation, sick leave, holidays, etc.
- Runs daily exception reports and corrects errors as needed.
- Maintains compliance of assigned patients’ visit frequency orders.
- Assists with internal transfer of patients between branch offices.
- Verifies visit notes as needed.
- Maintains accurate patient and employee information in the Electronic Health Record (HER) systems.
- Follows up and corrects any scheduling or care coordination issues reported by On-Call Clinician during after-hours operations.
- Schedules training visits for all new clinicians and ensures completion of on-boarding process.
- Ensures utilization of all per diem clinicians; reports non-utilization to HR as necessary.
- Maintains staff and patient statistics and reports.
- Reads and adheres to and participates in the Branch's mandatory HIPAA/Privacy Program and Employee Compliance Program.
- Reads and adheres to all Branch policies and procedures and follows Employee Handbook guidelines.
- Performs other duties as assigned.
What you need:
- Bachelor’s degree or equivalent in work experience
- Advanced proficiency in Windows environment, with emphasis Outlook, Excel, and Word
Preferred Education and Experience:
- Social Worker, Human Services, Psychology, or Business concentration
- 1 year experience in home health environment
