What are the responsibilities and job description for the Targeted Service Coordinator - Idaho Falls position at Innovative Health Care Concepts Inc.?
Innovative Health Care Concepts (IHCC) is seeking energetic, self-motivated individuals for our Targeted Service Coordination program in our Idaho Falls office. We are looking for people who can manage multiple tasks, foster relationships with clients and the community, and have effective communication skills.
This is a professional level position working closely with people with intellectual and developmental disabilities. Our current opening can be full-time or part-time with an opportunity to grow.
Applicant must have at least a Bachelor’s degree in a human services field. Previous experience working with individuals with developmental disabilities is preferred but not required.
Hourly wage is paid per billable hour
Full-Time is 30 Billable hrs/wk
Part-time is 10 - 29 Billable hrs/wk
Training wage: $22/hr
Billable hour wage: $23 - 26/hr depending on experience
Job duties include:
- Monitoring and coordinating services as requested
- Developing service plans and assessments
- Participating in monthly team meetings with other providers
- Documenting services provided and completing required paperwork on time and accurately
- Traveling to client homes and provider agencies
- Attending monthly staff meetings and trainings
