Fresno Program of All-Inclusive Care for the Elderly (PACE) is dedicated to providing its participants with comprehensive health and social supports that are proven to effectively manage chronic conditions and to reduce the risk for premature institutionalization. PACE staff are leaders in the “aging in place” industry and we have had the honor of serving Fresno, Bakersfield and Orange County seniors and their families/caregivers.
Job Summary
Under the supervision of the Activities Manager, assists in planning, developing and implementing appropriate, therapeutic recreational activities for PACE participants.
Essential Job Functions
• Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual
assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff on a daily
basis.
• Provides feedback to IDT Manager regarding care plan intervention outcomes.
• Coordinates group and individual activities that address the participants’ cognitive, physical, social, psychosocial, spiritual,
and artistic needs.
• Develops activities that include exercise, arts and crafts, birthday and holiday celebrations, special events, recreational
games, cognitive groups, and community outings.
• Coordinates between other operational areas that include but are not limited to transportation, nutrition services, social
services, nursing, and rehabilitation staff.
• Complete timely documentation, including daily participant activities participation records; initial, semi-annual, unscheduled,
and annual assessments; and other data on participants as directed by the interdisciplinary team.
• Assists with collection of PTP consent forms when photos are requested.
• Plan and prepare activities for dementia-specific population and with daily flow of quiet room activities.
• Supports Activities Manager to remain in compliance with assigned duties.
• Maintains confidentiality of participant information and reports any concerns or issues directly to Activities Manager.
• Provide coverage for other members of the activities department as needed.
• Participates in Activities Department meetings.
• Assists Activities Manager to ensure that all supplies and equipment are available to carry out recreational therapies.
• Prepares and submits inventory report to Activities Manager to assist with appropriate procurement and cost-effective
budgeting.
• Assist with the development of Recreational Activities Satisfaction Surveys.
• Assists with the preparation of activities utilization report.
• Assists Activities Manager to provide training and guidance to Activities Assistants.

• Attend and participate in staff meetings, in-services, projects, and committees as assigned.

• Be flexible in schedule of hours worked.
• May require use of personal vehicle.
Knowledge, Skills and Abilities
• Sufficient knowledge of activities/recreational therapy techniques and practices of the elderly in order to support the
Activities department.
• Advanced knowledge of the principles and techniques involved in relating to frail elderly persons.
• Creative abilities to plan, develop, and implement varied activities to meet the diversified participant needs
• Ability and experience working with volunteers and vendors.
• Skill in communicating with participants, family members, and staff; ability to establish a helping, trusting relationship with
participants.
• Ability to work effectively and harmoniously with staff, elderly, public and governmental agencies, providers of services, and
peers.

• Ability to access all areas of the Day Center throughout the workday.
• Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of
another qualified employee when attempting to lift or transfer objects over 25 pounds.
• Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm
movements, occasional bending, reaching forward and overhead; squatting and kneeling.
• Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
Requirements
Experience
• Minimum of two (2) years of demonstrated successful experience in activities/therapeutic recreation related services with
geriatric population.
• Minimum of one (1) year of documented experience working with a frail or elderly population.
Education and Certification
• At a minimum, a High School Diploma or GED required.
• Experience in planning and hosting events.
• Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant
contact.
