Job Summary
The Intake Specialist performs a variety of administrative duties, including completing, filing, and submitting participants intake forms. This position requires skill in planning, organizing and coordinating enrollment and retention activities, as well as the coordination of day-to-day administrative activities.
Essential Job Functions
Duties include, but not limited to :
- Answers phone calls from prospective enrollees and provides information regarding eligibility requirements and services.
- Schedules face to face meetings with prospective enrollees to conduct intake and to provide a site tour.
- Completes intake assessments with prospective enrollees and verifies Medi-Cal and Medicare coverage.
- Updates PrimeSuite and PaceLogic with new enrollment demographic data,
- Schedules Level of Care assessments and other pre-initial assessments as required.
- Completes enrollment conference with prospective enrollees.
- Updates Intake Tracking Log on daily basis for enrollment reporting to VP of Business Development.
- Responsible for meeting target enrollment and Business Development goals.
- Updates New Enrollee Logs monthly and other tracking logs as necessary.
- Maintains intake files and reports.
- Coordinates requests for medical records, working closely with the medical records department, for newly enrolled participants
