Job Title: Master Social Worker
Social workers play a vital role in helping patients navigate the complexities of End Stage Renal Disease. Their primary responsibilities include assessing patients' needs, educating them on care planning, and collaborating with healthcare teams to ensure comprehensive support.
Responsibilities:
- Assess patient needs and develop personalized care plans
- Educate patients on disease management, treatment options, and self-care techniques
- Collaborate with interdisciplinary teams to coordinate patient care
- Provide emotional support and counseling to patients and their families
Requirements:
- Master's degree from an accredited school of social work (MSW)
- Current state licensure (e.g. LCSW, LMSW), requirements for the state of practice
- Meet all state required regulations to practice in a dialysis setting
- Two years of clinical work experience, preferably in an ESRD setting
- Current CPR certification preferred
