Overview: The Care Coach at Innovative Senior Solutions plays a critical role in managing client relationships, providing case management, and navigating the referral process from initial contact to admission. This part-time position (20 hours per week) requires a combination of fieldwork and office responsibilities, focusing on building relationships within the community and ensuring a seamless transition for potential clients into our senior care services. The Care Coach is the direct representative for each client, overseeing the entire referral and admission process.
Key Responsibilities:
- Case Management: Provide comprehensive case management, ensuring potential clients are guided through every step of the referral process, from initial inquiry to formal admission into our care program.
- Community Relations: Establish and maintain strong relationships with community partners, including senior housing facilities, hospitals, personal care homes, and healthcare agencies.
- Field Work: Spend at least three days per week in the field meeting with potential clients in their homes, visiting senior housing, hospitals, personal care homes, and healthcare agencies. Promote services through Bingo on the Go's, case management visits, and ISS-hosted events.
- Client Admissions: Coordinate the complete admission process for new clients, including the signing of paperwork, scheduling home visits, and ensuring all required documentation is submitted.
- Tours and Day Passes: Organize and coordinate tours of our facility and offer free day passes to prospective clients and their families.
Office Work: Dedicate two office days per week to enter contact information, updates, and interactions into the CRM for potential clients and marketing contacts. Ensure that all interactions and follow-ups are properly documented.
CRM Management: Ensure accurate, up-to-date information in the CRM system and maintain clear communication with potential clients throughout the referral process.
Event Hosting: Organize and host informational sessions (ISS events) to engage potential clients and community partners.
Weekend Availability: Be available to work weekends as needed to accommodate client schedules and host events.
