We're thrilled to announce a dynamic opportunity for you to become an integral part of our Houston, Texas team. As an onsite contributor, you'll play a key role in pioneering a pathway toward a more streamlined and effective healthcare system.

This position requires travel to our Meyerland and Southbelt Clinics. The addresses of both clinics are provided below :

Meyerland - 10101 S. Post Oak Road, Houston, TX 77096

Southbelt - 12823 Gulf Freeway, Houston, TX 77034

JOB SUMMARY :

At Innovista Medical Center, the Care Coordinator is a vital member of the multidisciplinary team, working under the direction of the Care Manager. They handle health service coordination and clerical tasks, allowing licensed professionals to focus on complex clinical interventions. This approach improves patient outcomes, workflow efficiency, and satisfaction. The Care Coordinator collaborates with the clinic team, providers, caregivers, insurance, and community stakeholders to ensure seamless patient care and communication. This role is primarily based in an outpatient setting, with flexibility to assist other sites as needed.

DUTIES AND RESPONSIBILITIES :

  • Conduct initial and pre-visit patient outreach, including health risk screenings, assessments and patient-centered care plans.
  • Assist in scheduling appointments, arranging transportation, and ensuring patient compliance.
  • Support Care Managers and LCSWs with care coordination prioritizing patients for case management as needed. Also, conducting follow-ups based on patient needs.
  • Provide evidence-based educational promotion from the center’s content library and assist in patient education regarding preventive care and services.
  • Facilitate transitions of care by coordinating with the patients, caregivers, medical providers, and community resources.
  • Collaborate with the interdisciplinary care team (ICT) to ensure effective communication and coordination of care across various providers and community organizations.
  • Enter and maintain patient data, including relevant clinical and non-clinical data into medical management systems.
  • Managing, participating and coordinating Transition of Care (TOC), Complex Chronic, ED Diversion, Out of Network Steerage and Gap Closure.
  • Assist with reports, including call reports, census reports, and productivity reports, and support special projects related to case management and care coordination.
  • Participate in center meetings, marketing outreach activities, and contribute to quality improvement initiatives.
  • Requirements :

  • High school diploma or equivalent required.
  • 2-3 years of experience as a Care Coordinator in a health care setting.
  • Strong knowledge of medical terminology, preventive diseases, customer service, and complex chronic disease program are highly required.
  • Bilingual in English and Spanish required.
  • Epic EMR system knowledge is highly preferred.
  • Ability to navigate to other clinic location is required.
  • CPR / BLS certification required.
