Overview
We are seeking a dedicated and compassionate Care Coordinator Supervisor to join our team. The ideal candidate will play a crucial role in the organizaton's coordination of patient care, ensuring that individuals receive the necessary medical support and resources. This position requires strong organizational skills, attention to detail, and the ability to communicate effectively with both patients and healthcare professionals. The Care Coordinator Supervisor will work closely with various departments to streamline processes and enhance patient experiences within our medical office.
The Care Coordinator Supervisor oversees all Care Coordinator staff ensuring department excellence as they help patients and their families while in our care. A Care Coordinator helps with treatment planning, aftercare planning, family support and education. This includes coordinating transitions through levels of care and engaging outside resources to facilitate integration into life in early recovery.
Roles and Responsibilities
- Monitor and enforce patient conformance of program schedule, rules, and expectations
- Resolve conflicts through general and supportive advice
- Prepare and facilitate educational and motivational lectures and groups
- Act as a patient liaison in matters not limited to financial, legal, employment, custodial, and benefits including FMLA, STD and COBs while in treatment
- Coordinate with benefit services department for collection of patient funds related to housing, transportation, and copays
- Work with the treatment team, third party payors, families/loved one(s), etc. to facilitate transition throughout the continuum of care
- Coordinate services throughout the continuum, communicating pertinent information and plan of care to appropriate parties
- Collaborate with treatment team to encourage engagement in treatment and help facilitate the development of skills needed for long term recovery
- Ensure patient ROI’s are accurate, appropriately vetted and up-to-date
- Facilitate collateral, education, visitation and weekly family contact
- As needed, facilitate post admission interventions between patient and family
- As needed, carry a caseload
- Act as a part of the AMA blocking process and crisis intervention
- Participate in the approval process of patient transportation needs
- Coordinate with operations staff for all approved transportation requests
- Actively participate in documentation training and peer reviews
- Responsible for thorough, accurate, compliant and timely patient advocacy through documentation
- Observe, re-direct and report patient behavior
- Orient new patients and continuously enforce facility rules and regulations
- Build positive relationships with patients and staff
- Demonstrate adaptability and flexibility to changes in the work environment
- Report any safety/risk issues immediately to leadership and complete required incident report
- Be familiar with emergency management procedures
- Be compliant with directives from management
- Required trainings and certifications must remain active and in good standing
- Enforcement of policy and patient handbook as written and adopted at all times, without exception
- Maintain appropriate boundaries with patients and staff reporting any unsafe or questionable behaviors
- Any other duties as assigned
