Job Summary
Provide administrative support to the Service Coordinator in delivering member services, including non-waiver and waiver services. Assist with member contact, education, and coordination of community resources to promote optimal health.
Essential Duties and Responsibilities
- Answer incoming faxes, emails, and phone requests.
- Initiate daily member phone calls, identify issues, and refer to the Service Coordinator or other departments.
- Schedule, cancel, and document appointments for face-to-face and telephonic requests.
- Receive and document initial faxes, emails, or phone requests, and refer to appropriate personnel as needed.
- Screen and evaluate calls, referring as necessary.
- Gather data for statistics and maintain logs.
- Manage intake of MNLOC and ISP requests.
- Monitor portal for timely completion of assessments and reassessments.
- Communicate progress of MNLOC reviews.
- Monitor portal for approval or denial decisions on MNLOC / ISPs and document outcomes.
- Update due dates for ISP and MNLOC in the service coordination system.
- Complete eligibility and benefit data before routing to the Service Coordinator or Transition Specialist.
- Provide communication and education regarding case management and transition services.
- Educate members about managed care, benefits, and community resources.
