Day to Day:
- The Social Worker II provides a range of individualized services to patients with significant medical and cognitive disabilities and their caregivers. The Social Worker II is responsible for psychosocial assessments, identifying health-related social needs for each family on the assigned caseload. They are responsible for planning and implementation of the patient’s treatment and discharge plan, which reflects the patient’s social work needs from admission to discharge, and which requires added collaboration and coordination responsibilities.
Must Haves:
- LCSW or LICSW
- Masters in Social Work
- 1 years of clinical experience post-Masters
Hourly Pay Rate:
- $40-$60 (Depending on experience)
Schedule:
- 8:30 AM-5:00 PM
- Full-Time
Job Details:
- 6-8 month contract (Temporary)
