We are seeking an experienced Nurse Practitioner to join our Risk Adjustment team, specializing in end-stage kidney disease (ESKD) care. You will manage cases for patients with kidney disease, focusing on education, medical history, medications, support systems, lifestyle changes, and behavior modifications to reduce future medical costs through early identification and support for dialysis and transplant needs.
You will collaborate with nephrologists for rounding and annual assessment visits in dialysis centers, with travel to centers and practice offices. The role may also include telehealth (20%) and home visits (10-20%) as needed.
Schedule: Flexible, M-F 8 hour day shifts
Must Haves:
- Current specialty certifications and license to practice as an Advanced Practitioner in the state Interested (New York)
- Enrolled in CMS (Medicare) of the state of New York.
- Firm understanding of the protocols, policies, and operations of a physician’s practice
- Minimum of 2 years of Nurse Practitioner experience
- Experience working in Nephrology and/or internal medicine specialties (e.g., cardiology, endocrinology)
- Comfortable traveling between facilities and working independently

Responsibilities are listed below:
- CKD Clinical Care by offering clinical care and kidney disease education.
- Provides specialized care to patients with kidney diseases, manages dialysis treatments, and collaborates with physicians
- Risk Adjustment Documentation
- Conduct comprehensive assessments with a focus on risk adjustment documentation.
- Holistic Patient Care Management
- Manage end-stage kidney disease patients holistically, addressing activation, depression, hospitalization risk, transitions of care, and co-morbid medical issues.
- Review patient charts for chronic kidney disease (CKD) and make suggestions as needed
- Assist in referrals to IDT teams (referring case management or social worker follow-ups)
- Travel between multiple dialysis facilities and work independently
- Completing rounding to meet and treat patients
Salary : $130,000 - $135,000
