Insight Global is looking for skilled In Patient Case Managers to sit in our largest Healthcare client in NJ. These candidates will coordinate the care and services of selected member populations to facilitate a safe and timely transition to the next appropriate level of care. Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs. They will partner with the healthcare team to ensure all aspects of the patient's needs, clinical, psychosocial and financial, are adequately addressed to manage the patient's timely progression of care and safe transition from acute care.

Required Skills & Experience
- 2 years of experience working as RN Case Management i
- Certified in NJ- RN; BSN Preferred
- NICU / Cardiac / Med Surg specialty experience
