Medical Rec Assistant lll is responsible for verifying medical record completion, including reporting, fining and quality based upon standards established by TJC-HFAP CMS and the Medical Staff Rules and Regulations
This individual documents discrepancies in medical record completion including an explanation to provide feedback and education to interested parties
Responsibilities:
- Analyze each ambulatory surgery record (within 5 days of discharge) according to departmental procedures with 95% accuracy
- Analyze the medical record and carefully determine which reports need to be dictated and/or signed by a physician
- Assign each deficiency in the record to the appropriate physician
- Check for any missing reports which are indicated as having been previously dictated prior to assigning a dictation deficiency
- Enter all deficiencies into the department's computerized record deficiency system according to the departmental procedure with 95% accuracy, and produce a deficiency slip for each admission
- Input or update the record deficiencies in the computerized record deficiency system correctly including the changing of chart date when appropriate
- Print a deficiency slip, and attach it to the admission color tag all items requiring signature within the record
- File the record in the appropriate area within the department
- Perform other functions related to discharge analysis according to departmental procedure as necessary
- On a daily basis, verify that all discharged records for a particular discharge day have been received in the department, and if necessary, successfully locate any that have not been returned
- Assemble the discharged record according to the department's order of the medical record guidelines as necessary
- Create medical record folders for reactivated microfilmed records or volume expansion record folders as necessary according to departmental guidelines
- Attach all types of medical reports to the appropriate discharged record according to departmental procedure and guidelines
- Notify supervisor of any registration information errors which need to be corrected in the hospital's computerized database
Minimum Required:
One year experience in a medical office setting to include clerical filing. Demonstrated experience in Microsoft Office Suite to include Excel.
