Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and in navigating complex systems to access necessary resources and services. The Case Manager will work closely with clients to assess their needs, develop personalized service plans, and provide ongoing support throughout their journey. The Case Manager will also have some clerical roles including scheduling appointments, answering the phone, and communicating with clients effectively.
Responsibilities
- Clerical responsibilities, strong computer skills preferred.
- Develop individualized service plans that address specific goals and objectives.
- Coordinate services with various agencies.
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Maintain accurate documentation of client interactions, progress notes, and case files.
Skills
- Strong background in social work or related fields.
- Proficiency in crisis management.
- Experience in addiction counseling is preferred but not required.
- Excellent communication skills.
- Ability to work collaboratively within a team environment.
- Strong organizational skills with attention to detail for managing multiple cases effectively.
