Access Care Coordinator, Bilingual Spanish and English Preferred – AKDHC Tucson St. Mary's Office

 


This Access Care Coordinator, Bilingual Spanish and English Preferred assesses, coordinates, and identifies the vascular access needs and services of CKD & ESRD patients. Provides ongoing monitoring of the patient’s vascular access to assure life of the access is achieved and plans for any intervention or monitoring will be achieved. This position is responsible for assisting Surgical and Nephrology schedulers with interventional and permanent access placement. The primary goal is to reduce the prevalence of Hemodialysis catheters in new ESRD and chronic ESRD patients. This position is direct back up to the surgical and nephrology scheduler. 

 

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Liaison between AKDHC and Dialysis units as well as outside Surgeon’s offices 
  • Participate in access education, early planning, and scheduling of vascular access procedures. 
  • Monitors all ESRD reports, CKD reports and QA reports from administration and dialysis units. 
  • Reviews patient’s current access status and collaborates with office and dialysis unit’s clinical staff in developing a plan of action for permanent access. 
  • Coordinates timing of access placement based on Stage/GFR and patient need for dialysis. 
  • Ongoing monitoring of the patient's permanent access: communicates with staff in making sure any US, Limited Studies and/or Interventions needed are scheduled. 
  • Receives calls/faxes from patients, dialysis units or physician offices needing to schedule patients for procedures, vessel mapping, ultrasounds, and permanent access placement. 
  • Notifies the Nephrology and Surgical schedulers of any appointments/procedures that need to be scheduled. 
  • Assists to schedule patients to outside vascular surgeons according to patient’s access plan and insurance. 
  • Follows-up with patients regarding surgical plan and provides education to patients that are being referred outside of AKDHC for access placement. 
  • Does direct follow up with all outside surgeons on patient appointments and surgeries. 
  • Serves as an educational resource for office and dialysis unit staff regarding vascular access. 
  • Attends and participates in monthly FMC Catheter Conference calls and other committee meetings to represent the Access Center within Arizona Region: Participates on committees as directed. 
  • Monitor Physician Rounds/QA on a daily basis. 
  • Request and review all dialysis unit flows studies on a Monthly basis. 
  • Track and monitor patient referrals and scheduled appointments. 
  • Maintains access history and monitoring if patients are being seen on a routine basis. 
  • Updates and keeps current AKDHC EMR programs related to access information. 
  • Keeps supervisor informed on any patient issues, and overall opportunities for improvement. 
  • Partners with FMCNA Clinical and Operations team to provide, coordinate and integrate 
  • education on hemodialysis access types with focus on the health and safety benefits of 
  • AV-Fistula or AV-Graft compared to central venous catheters. 
  • Coordinates the Nephrology Practice late stage CKD patient population with regard to CKD education, including modality selection, permanent access placement and maintenance and a stable transition to RRT with an expected patient case load of 500-700 patients. 
  • Assesses patients’ knowledge of late stage CKD and treatments, educating and informing patients to enable them to make informed decisions regarding the creation and maintenance of a permanent dialysis access for patients starting RRT. 
  • Adheres to strict patient and corporate confidentiality.

  • Education/Experience: High school diploma or general education degree (GED); or one to three years related experience and/or training; or equivalent combination of education and experience. 
  • Language Ability: Bilingual capabilities speaking, reading, and writing Spanish and English highly preferred; Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. 
  • Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. 
  • Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 
  • Computer Skills: To perform this job successfully, knowledge of Microsoft Office software, Spreadsheets, E-Mail, and Internet Software Is helpful. 
  • Certificates and Licenses: Basic Cardiac Life Support
