Job Description
Responsibilities and Duties
The work of a Direct Support Professional is directed by a plan of care developed by the Oregon Department of Human Services. The Direct Support Professional does not organize the care plan but is responsive to it in providing care and support to the client.
Position Expectations (may include, but are not limited to the following:)
- Cooking, cleaning, medication assistance and reminders, scheduling, attending medical appointments, personal care and hygiene, budgeting, housing, transportation
- Skill development and support as defined in the Individual Support Plan (ISP)
- Documentation of progress notes at the end of each shift
Qualifications and Education Requirements
- High School Diploma or GED equivalent
Preferred Skills
- Experience providing general care and support
- Knowledge of home care services and supports
- Proficient in written and oral communication
- Overall competency in working under pressure
General Staff Qualifications: A staff member delivering services to an individual must meet the following criteria:
- Be at least 18 years of age
- Be legally eligible to work in the United States
- Hold a current, valid, and unrestricted professional license or certificate where services and supervision require specific professional education, training, and skill
- Understand requirements of maintaining confidentiality and safeguarding individual information
- Not be on the list of excluded or disbarred providers maintained by the Office of the Inspector General of the State of Oregon
- Be literate and capable of understanding and following both written and oral orders
- Be able to communicate with individuals, health care providers, case managers and others as appropriate
- Be certified in CPR / First Aid by a recognized training agency within 60 calendar days of being listed as a employee
- Receive 24 hours of job-related in-service training annually
- Be clear as to the job responsibilities as described in a current, signed and dated job description
- Have a working and reliable means of transportation
- Maintain a current and valid driver license
- Maintain valid and current automobile insurance, with at least liability coverage
- Additional qualifications as required by applicable endorsed agency staff program rules

Pay: $23.00 - $24.00 per hour
