Inspire Home Health is looking for a dynamic registered nurse to lead our growing patient needs. Our location in Farmington, MO sits wonderfully within our network. This growth opportunity will be a great addition to a nurse looking to expand leadership skills . Full-time days consisting of being in the office in Farmington with the occasional home visit needed to lead the team towards excellence in home care.

Position Summary: 

RN Clinical Manager is a qualified full-time employee appointed by the Governing Body to direct and supervise professional and auxiliary personnel providing home health services. The RN Clinical Manager will be responsible for the direction, coordination, and supervision of home health services.

Essential Functions:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Follow policies and standards established by the Governing Body, and under the supervision of the Administrator.
  • Assist with planning, coordination, leading, and evaluation of home health services.
  • Responsible for coordination of care with all agency departments, clinical team, committee’s referral sources, and community partners.
  • Consults, plans, and works with Administrator in policy and program development.

Duties / Responsibilities:

  • Consults with leadership and staff, regarding services provided to patients.
  • Evaluates the services provided and clinical team.
  • Lead and coordinate Weekly Case Conference Meeting
  • Schedule – weekly nurse schedule; monthly on call schedule; input on call scheduled into on call service portal
  • Manage Clinical System - Point Click Care (PCC) Dashboards
  • Monitor / complete coding portal and Quality Assurance process
  • Directs the implementation of the individualized plan of care, including goals and coordination of services.
  • Develops standards to ensure safe and effective services are provided to patients.
  • Establishes and maintains effective channels of communication with patients, the patient’s responsible party, leadership, clinical team, referral sources, and community partners.
  • Ensures staff development, including orientation, in-service education, continuing education, and competency evaluation.
  • Directing and monitoring performance improvement activities, including Quality Assessment and Performance Improvement (QAPI).
  • Complete patient visits as needed
  • Assist with staff orientation and training
  • Follows and implements policies and procedures.
  • Ensures policies and procedures are maintained and updated per regulations.
  • Participates in staff meetings and committees, as indicated per leadership team.
  • Follows, implements, and updates appropriate agency documents, leadership, and agency personnel regarding federal, state, and local regulatory practices.
  • Ensure standards of ethical business and clinical practice are maintained.
  • Direct staff in performance of their duties Adheres to the Americare Mission/Vision, Home Town Hospitality and F.A.M.I.L.Y. Values, Employee Culture Statement and Company Policy and Procedures
  • Completes other duties as assigned by the leadership team.

Qualifications/ Education/Experience/Skills

Education:

  • Must be a Registered Nurse.
  • Minimum of one year of home health experience.
  • Must have at least one year of supervisory or administrative experience in home health or related health programs.
  • Must meet initial and ongoing educational requirements as specified by the Federal and State regulation.

Licenses/Certifications :

  • Current state Registered Nursing (RN) license
  • Must meet initial and ongoing educational requirements as by the State.
