Physical Therapist Home Health Care

Key Responsibilities :

• Patient Evaluation :
• Conduct initial OASIS evaluations and assessments of patients in their homes to determine their physical therapy needs.
• Review medical histories, diagnoses, and physician orders.
• Identify patient-specific goals and develop individualized treatment plans based on assessments and appropriate service utilization offering cost-effective treatments that help improve clients' motion and mobility
• Reduce the need for medications and provide alternatives to surgery
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients' specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
• Consult and practice with other health professionals
• Evaluate effects and monitor and communicate progress
• Document patient care services
• Treatment Implementation :
• Develop and implement treatment plans to help patients regain mobility, strength, and functionality.
• Educate patients and their families on exercises, techniques, and safety measures to assist in the healing process.
• Utilize various physical therapy modalities (e.g., manual therapy, exercises, therapeutic equipment, etc.).
• Progress Monitoring :
• Monitor and document patients' progress, adjusting treatment plans as necessary.
• Communicate regularly with physicians and other healthcare professionals to report on patient progress and updates.
• Patient Education :
• Teach patients and caregivers techniques for proper body mechanics, injury prevention, and at-home exercise programs.
• Provide guidance on the safe use of adaptive devices or equipment, such as walkers or braces.
• Collaboration :
• Work closely with other healthcare team members, including physicians, occupational therapists, nurses, and social workers, to ensure comprehensive care.
• Attend multidisciplinary meetings and provide input on patient care plans.
• Documentation and Compliance :
• Maintain accurate , up-to-date patient records in compliance with HIPAA regulations and company policies.
• Complete all required documentation for billing, insurance, and patient records.
• Travel :
• Travel to patients' homes to deliver services, which may include driving long distances or managing a caseload spread across a wide area.

Salary : $120,000
