Day to day as a Home Health - Wellness Nurse/Visiting Nurse
As a Home Health - Wellness Nurse/Visiting Nurse with Inspired Homecare, your day will be filled with variety and meaningful interactions! You'll kick off your morning by reviewing client care plans and setting personalized wellness goals. Expect to spend your day visiting clients in their homes, providing compassionate assessments, and delivering tailored health education.
Your role will involve administering medications, monitoring vital signs, and identifying any changes in health status to ensure the best outcomes. You'll also collaborate closely with family members, caregivers, and other health professionals to create a supportive care environment. With a focus on customer-centric care, you'll be empowered to problem-solve and respond to your clients' unique needs, all while using your empathy and integrity to uplift their spirits.

What you need to be successful
To thrive as a Home Health - Wellness Nurse/Visiting Nurse you'll need a mix of technical skills and heartfelt compassion! Previous Home Health experience is essential, along with familiarity using EMR systems like HCHB and WellSky Kinnser. You'll be available and willing to travel to multiple clients across McHenry, Lake Zurich, and surrounding areas, ensuring every patient receives the care they deserve. Your assessments will be critical; expect to perform evaluation tasks, including vital signs checks, medication reviews, Oasis assessments, wound care, and IV therapy. You'll dress or redress open wounds while tracking the healing process and documenting your observations to meet company and compliance standards.
You'll supervise CNAs and LVNs on your caseload while educating patients and their families on effective home health care strategies. Coordination with Occupational and Physical Therapists, Physicians, and others involved in care plans is key, along with making thoughtful recommendations to enhance patients' quality of life. Always remember-listening to your patients and addressing their concerns is at the heart of what makes you successful!
Knowledge and skills required for the position are:
- Previous Home Health Experience required
- Experience using EMR: HCHB and Wellsky Kinnser
- Available and willing to travel to multiple locations
- Assess patients' condition during every visit and chart your observations
- Perform evaluation tasks
- including vital signs and medication review
- Oasis assessment
- wound care and IV therapy.
- Dress or redress open wounds and assess progress of healing
- Collect data and prioritize problems and needs
- Supervise CNA and LVN under assigned to your caseload
- Document to company and compliance standards
- Educate patients and their families on proper home health care strategies and procedures
- Coordinate with Occupational Therapists
- Physical Therapists
- Physicians and anyone else involved in the patient's care plan
- Make recommendations for devices or tools that might improve the patient's quality of life
- Listen to the patient and respond to concerns or request
