Job Description:
The Social Worker 2 will work as part of the Clinical Care Team, supporting referrals from primary care providers and collaborating closely with nurses and providers to address social determinants of health (SDOH). This role involves direct patient engagement, resource coordination, case management, and ongoing support to ensure patients access needed services and care.
Key Responsibilities:
Social Support Navigation:
- Conduct in-person or remote assessments for social needs (e.g., housing, food insecurity)
- Maintain updated resource lists with eligibility, contacts, and referral processes
- Connect patients with community-based resources
- Collaborate with other social workers and care team members
Case Management:
- Coordinate discharge planning and follow-up care with hospitals
- Assist in securing medical equipment through community partners
- Obtain medical records from hospitals and specialists
- Identify and re-engage patients lost to follow-up
Specialty Referral Navigation (as needed):
- Schedule and track specialist and imaging appointments
- Maintain a database of specialty referral options and processes
- Assist in record retrieval from imaging centers and specialists
Prescription Support:
- Help patients access low-cost or free prescription options
- Assist with patient assistance program (PAP) applications (e.g., CoverRx, RxOutreach)
General Support:
- Support other regional primary care-based initiatives with a social work component
- Provide or locate appropriate patient education materials
- Document all activity in the patient record and notify relevant clinical staff
Additional Expectations:
- Complete onboarding and orientation
- Attend regional office and primary care team meetings
- Participate in provider and community meetings to build partnerships
- Identify and address patient barriers to care

Pay: $27.57 - $30.00 per hour
