About the Role
The Regional Engagement to Advance Community Health (REACH) Partnership is an effort improve health outcomes and social determinants that may prevent individuals from living their best life. REACH deploys community health workers (CHWs), community paramedics (CPs), and AmeriCorps members in partnership with multiple stakeholder agencies, creating the potential for coordinated care teams and leveraging resources.
In alignment with IALR’s mission to drive economic transformation, this role provides coordination, data reporting, partner communication, and program support for both REACH and the REACH AmeriCorps program. The Coordinator also assists with grant reporting, training coordination, outcome tracking, and public presentations, serving as a key liaison between IALR and regional partners to ensure effective program delivery and community impact.
Duties & Responsibilities:

Lead coordination meetings with care teams to review caseloads, develop care plans, identify at-risk clients, and streamline discharge and service delivery.

Maintain compliance across data systems by ensuring accurate documentation, timely updates, and organized access to reporting and program materials.

Collect and analyze program data from REACH partners; generate reports using approved dashboards and visualization tools.

Review and process reimbursement requests from partners, including follow-up on required documentation.

Support grant writing and funding opportunities, and serve as a liaison between the REACH Program Manager and partner organizations.

Draft quarterly and annual reports, coordinate communications and training, and assist in the promotion of REACH and AmeriCorps initiatives.

Support AmeriCorps recruitment, onboarding, supervision, and service activity coordination.

Represent IALR and the REACH Partnership in community meetings, events, and public presentations.

Collaborate across divisions to support IALR’s mission and strategic goals, and perform other duties as needed.
Work Location
We are looking for staff to work onsite in Danville, VA.
The Ideal Candidate

Bachelor’s degree and at least four years of experience in education, business, or a customer-facing role, preferably in healthcare.

Proven experience in program coordination, data tracking, and financial reporting.

Proficient in Microsoft Office and virtual collaboration platforms, with strong computer literacy.

Excellent organizational, communication (written and verbal), and public speaking skills.

Ability to manage multiple tasks, meet deadlines, and adapt in a fast-paced environment.

Demonstrated ability to work independently and as part of a team, with strong interpersonal skills.

Familiarity with regional resources and a commitment to IALR’s mission.

Valid driver’s license and willingness to travel throughout Virginia and occasionally out of state.
Preferred Qualifications:

Experience with grant writing, developing MOUs, and program evaluation for outcomes and strategic improvement.

Familiarity with EMR systems, CRM tools, or other program evaluation software.

Knowledge of federal, state, and local funding resources.

Experience promoting or marketing educational or population health programs.

Salary & Benefits
IALR will offer a salary of $58,000 - $64,000 plus applicable shift differential.
