Primary Care Physician Opportunity
In Clarksville, we have a wonderful opportunity for a lead Primary Care Physician to join our team-based care environment.

Our multidisciplinary care team includes doctors, advanced practice professionals, pharmacists, care coaches, nurses, medical assistants, behavioral health specialists, quality-based coders, referral coordinators, and more.
Responsibilities
- Evaluate and treat patients in accordance with established standards of care.
- Monitor patient care using available data and chart reviews, ensuring the highest level of medical quality.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
- Participate in daily huddles as an active member of the care team, providing medical expertise and collaborating with colleagues.
- Work closely with the regional medical director and center administrator to promote a culture of cooperation and professionalism.
- Complete accurate and timely medical record documentation, working with a quality-based coder to optimize coding specificity.
- Adhere to policies and protocols defined by clinical leadership.
- Meet with the regional medical director to discuss quality of care, review outcome data, policy, procedures, and records issues.
- Pursue opportunities for growth and development within the center, exploring new or existing services.
- Participate in the local primary care on-call program as needed.
- Maintain compliance with licensing, certification, and accrediting bodies requirements.
- Dedicate 100% of your time to direct patient care, including patient-facing interactions and general administrative tasks related to patient care.
Required Qualifications
- Unrestricted medical license in the state of practice, with the ability to obtain licenses in other states within the region as required.
- Graduate of an accredited MD or DO program from a reputable university.
- Excellent verbal and written communication skills.
- Highly developed interpersonal relationship and communication skills with colleagues and patients.
- Strong commitment to integrated team-based care.
- Ability and willingness to adapt to a value-based care setting.
- Superior patient/customer service skills.
- Basic computer skills, including email and EMR systems.
- This role is considered patient-facing and involves participation in the Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.
