We are seeking a highly skilled Primary Care Physician to join our team-based care environment in New Albany, IN. As a key member of our team, you will be responsible for providing high-quality patient care and collaborating with other healthcare professionals.
">- Assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
- Complete all medical record documentation in a timely manner working with a quality-based coder to optimize coding specificity.
- Follow policy and protocol defined by Clinical Leadership.
- Participate in potential growth opportunities for new or existing services within the Center.
Qualifications:
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Knowledge of Medicare guidelines and coverage.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
