Nurse Practitioner / Physician Assistant opportunity in Lake Havasu City, AZ

Exceptional Nurse Practitioner / Physician Assistant opportunity offering flexibility and the opportunity for career growth as a clinician. As a nurse practitioner (NP) or physician assistant (PA), you’ll play a critical role in delivering high-quality, safe, and cost-effective patient care in our local post-acute care communities, supported by our dedicated local and regional operations team.

In our facilities, you’ll work as part of an interdisciplinary team, managing all aspects of patient care, including admission, progress, transfer, discharge, and follow-up visits. Additionally, you’ll coordinate with the facility to transition patients to the most suitable care setting upon discharge, whether that’s returning home, an independent living center, assisted living, or memory care. Our team collaborates closely with hospice, palliative, and home care agencies to ensure safe transitions.

Our productivity-based compensation model (Fee for Service) empowers you to maximize your income potential, with an estimated annual range of $132,000 to $158,000 and no cap on productivity income. This full-time position is also eligible for benefits.
