Adult Health Home Care Manager - Full Time - Flexible

JOB TITLE: Adult Health Homes Care Manager
CLASSIFICATION: Full Time, Non-Exempt
PROGRAM/DEPARTMENT: Health Home Care Management
IMMEDIATE SUPERVISOR: Program Manager

POSITION SUMMARY: Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy.

QUALIFICATIONS:Minimum Education, Training & Experience:
Bachelor’s Degree in Human Service related field
Or
Credentialed Alcoholism and Substance Abuse counselor (CASAC)
And
Two years of experience providing Care Management services (Case Management) to individuals with qualifying factors; i.e. physical/MH/ Substance diagnosis

Preferred Education, Training & Experience:
The above plus two (2) years’ experience developing Care Plans

ESSENTIAL SKILLS AND ABILITIES:
Valid New York State driver’s license and dependable transportation that meets the Agency’s driving policy
Ability to work flexible hours and travel throughout Cattaraugus, Allegany and Chautauqua Counties
Ability to work effectively and independently including address multiple tasks efficiently
Possesses strong organizational, analytical and creative thinking skills
Ability to effectively communicate verbally and in writing including developing rapport with the people we serve
Ability to represent self and agency in a professional manner
Possesses strong knowledge of community resources
Willingness and ability to participate in an on-call rotation

SALARY:
$21 per hour starting rate
